How do you keep a down blanket from slipping around inside a duvet?

After I put my comforter inside the duvet and actually used it, the comforter slipped away from the duvet and bunched up in one big ball! Help!

First of all, are the buttons on a duvet supposed to go up by your head, or by your feet?
Also, how the heck do you stop the comforter from moving around inside the duvet cover? It’s so annoying!
